A Form is a electronic document that contains blank spaces or fields.  A form is also commonly referred to as a window or screen.   These blank spaces or fields provide the user with a entry point which they can use to view or enter information into the computer.

The issues form shown below is an example of a form.  The field labeled Company is an example of a data entry point.
